Prevention of Future Death Reports
A Prevention of Future Death (PFD) report is written by a Coroner after an inquest, especially where the Coroner has heard evidence that further avoidable deaths could happen if preventative action is not taken123. The report is sent to the person or authority who have the power to make the changes that are suggested. They have to respond to these within 56 days showing how they have made changes according to the Coroner’s recommendations, or how they intend to make them.

Version one of the PFD data is England and Wales only and there are a lot of PDF reports that will need to be extracted to give you the recommendations, although the higher level data is captured.
